GOLF HOLE-IN-ONE INSURANCE
TERMS AND CONDITIONS

  1. Joseph Kovacs Insurance must be notified in writing prior to the event of any changes being made to the policy. Failure to notify Joseph Kovacs Insurance could result in a denied claim.
      

  2. The minimum yardage on any selected hole must be 150 yards from the tee to the pin. If the selected hole is more than 150 yards it cannot be decreased and must be played as stated on the score card. (135 yards for women will be acceptable)
      

  3. In the event of a rain out Joseph Kovacs Insurance must be informed in writing no later than one week from the tournament date.
      

  4. One tournament official must be stationed and monitoring play at each of the Insured Holes at all times during the Covered Tournament.
      

  5. The tournament official stationed at the Insured hole can be no younger than 16 years of age.
      

  6. If the event has 25 players or less a representative from the course must monitor the hole (for example, the course marshal, club pro, club manager, etc.).
      

  7. Applications must be received by Joseph Kovacs Insurance the day prior to the event. Applications with coverage over $35000 CDN must be received at least 24 hours before the event Monday to Friday.
      

  8. A Participant means an individual (not including Golf Club Pro or Professional Golfer) entered and playing in a Covered Tournament, for which premium has been paid.
      

  9. For Pro's to be eligible, the hole must be playing a minimum of 175 yards and up to Underwriters approval. Please contact Joseph Kovacs Insurance prior to the start of the event.
      

  10. Coverage is only applicable in the amount not exceeding the limit of insurance stated in the policy declaration. Insurance is for the cost incurred in providing an award to the participant whom, during the policy period, makes the "first" HOLE-IN-ONE on the selected hole(s) in a covered tournament.
      

  11. The applicant must inform Joseph Kovacs Insurance of a HOLE-IN-ONE within three working days following the event. (It is in the applicant's best interest to call from the golf course as soon as HOLE-IN-ONE occurs.)
      

  12. In the event of a HOLE-IN-ONE the Insured must get names and phone numbers of all members playing in the foursome, the original tournament scorecard and the name(s) and phone number(s) of all monitors. Joseph Kovacs Insurance will fax an affidavit and claim form as soon as they are informed of the HOLE-IN-ONE. This must be filled out as a soon as all information can be collected.
      

  13. The Participant must finish the entire round of golf and complete the scorecard.
      

  14. In the event of a HOLE-IN-ONE, certification of achievement must be made on the score card of the Participant achieving the HOLE-IN ONE by the tournament official monitoring play at the Selected Hole and all Participants within the group in which the HOLE-IN ONE was made.
      

  15. The Tournament must be played in groups of three or four. No twosomes are allowed.
      

  16. A ball hit out of bounds and replayed does not qualify as a HOLE-IN-ONE. This would constitute a HOLE-IN-THREE. Mulligans are not eligible for a HOLE-IN-ONE.
        

  17. Joseph Kovacs Insurance will indemnify the first hole in one sponsor only, to a maximum of $35,000 CDN, for each covered hole. Check with the tournament organizer to ensure that you are the only sponsor on the hole.
